We understand the importance of ensuring that your benefit goes to the people you hold dear. To make this process more seamless, we kindly request that you complete a beneficiary nomination form. This form will serve as a crucial document, allowing Unum to determine who should receive your benefit in the event of your death. It is essential to keep this form updated as any changes in your circumstances occur. Please complete the form and return it to your HR representative.

We kindly remind you that separate nomination forms need to be completed for both your Group Pension with Standard Life and Group Life Assurance scheme to ensure that your wishes are properly addressed.
