GROUP LIFE ASSURANCE
Included in your companys benefit package is the Group Life Assurance scheme, which is administered by MetLife. This scheme enables your employer to provide a tax-free lump sum benefit to your loved ones in the unfortunate event of your death. Farne Salmon & Trout has arranged to base this lump sum on a multiple of your salary, ensuring that your family & friends are taken care of financially during such a difficult time. Please note, that there are eligibility criteria for this benefit. Please check your contract, or your HR department, for further details.
We understand the importance of ensuring that your benefit goes to the people you hold dear. To make this process more seamless, we kindly request that you complete a beneficiary nomination form. This form will serve as a crucial document, allowing MetLife to determine who should receive your benefit in the event of your death. It is essential to keep this form updated as any changes in your circumstances occur. Please complete the form and return it to your HR department for safe keeping.
We kindly remind you that separate nomination forms need to be completed for both your Group Personal Pension and Group Life Assurance scheme to ensure that your wishes are properly addressed. You'll find the appropriate nomination form in respect of your Group Personal Pension on the previous page, or you can complete it via your account through your MyAviva login.

Where necessary, GP's can also issue private prescription for collection from a pharmacy or delivered to your door. If an NHS practising doctor considers symptoms require further investigation, a doctor can issue an open private referral letter to enable access to continued treatment.
